Plains Commercial has been retained as the exclusive marketing adviser for the sale of Hennessey Care Center (Hennessey) & Hensley's Nursing Home (Hensley's) located in Hennessey & Sayre, Oklahoma respectively (collectively the Portfolio). Offers will be evaluated based on price, terms, and investor qualifications.

The Portfolio offers investors a unique opportunity to acquire cash flow positive SNFs with strong upside through further increase in census. The facilities have a history of stable occupancy coupled with healthy reimbursement rates. Oklahoma has an attractive Medicaid rate with regular rate increases over the past few years. For 2024, Hennessey's Medicaid rate was approximately $235/day and Hensley's Medicaid rate was approximately $224/day according to the most recent rate letters.
